Your Exciting Role:

Lead and Support: Supervise and coordinate clinical services, providing quality support to patient-facing teams.
Provide Direct Patient Care: Deliver clinical services within your scope of practice, following the plan of care and collaborating with the healthcare team.
Manage Referrals and Assignments: Receive referrals, ensure appropriate clinician assignments, and evaluate patient orders.
Coordinate Patient Benefits: Determine patient home health benefits, medical necessity, and ongoing insurance approvals.
Ensure Quality Care: Continuously assess patient needs, individualize care, and ensure compliance with physician orders and eligibility criteria.
Review and Consult: Review assessments and plans of care daily and consult with clinicians as needed.

What You'll Bring:

RN Licensure: Current RN licensure in the state of practice.
CPR Certification: Current CPR certification required.
Driving Requirements: Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
