About the Role:

As a Project Coordinator for the Health Home Plus program, you will play a vital role in supporting individuals with Serious Mental Illness (SMI) and ensuring they receive the comprehensive care they need. This is a hands-on position where you will be directly engaging with clients, assisting them in overcoming challenges, and guiding them toward self-sufficiency.

Key Responsibilities:

Caseload Management: Carry and manage a caseload of up to 20 clients, conducting assessments, developing personalized care plans, providing service referrals, and offering crisis intervention when needed.

Field Work & Client Visits: Conduct in-person visits with clients at least twice per month to ensure progress. Additional field visits and telephonic check-ins may be required based on client needs.

Client Advocacy & Goal Support: Help clients overcome barriers to reaching their goals, assist with self-advocacy, and implement strategies to encourage full participation in the program.

Documentation & Monitoring: Track clients' progress through scheduled follow-ups, home and office visits, and by maintaining detailed service records, including progress notes and service plan outcomes.

Community Outreach: Assist in marketing the program and recruiting new clients from the community. Additionally, you may need to screen clients over the phone for eligibility and schedule appointments for potential enrollment.

Client Support: Assist clients in completing applications for benefits, entitlements, and other necessary services, ensuring they have the tools and support needed to thrive.

Hybrid Work Schedule: The role includes a hybrid work schedule of generally 3 remote days and 2 office days, with some field work required on remote days based on client needs. Office days may vary depending on program performance.

What We're Looking For:

Education and Experience:

Master's Degree in a related field with 1 year of experience; OR
Bachelor's Degree in a related field with 2 years of experience; OR
CASAC (Credentialed Alcoholism and Substance Abuse Counselor) with 2 years of experience; OR
Bachelor's Degree in any field with 3 years of experience or 2 years of experience as a Health Home care manager for individuals with Serious Mental Illness (SMI) or Serious Emotional Disturbance (SED).

Other Qualifications:

Experience providing direct services to individuals with SMI, developmental disabilities, substance abuse, or children with SED is preferred.

Strong communication skills are a must, including the ability to engage with clients who may be resistant or difficult to work with.

Ability to manage multiple priorities effectively and meet deadlines.

Comfortable using Microsoft Office (Word, Excel) and web-based reporting platforms.

Experience in conducting care conferences with medical providers from different specialties is a plus.

Compensation & Benefits:

Salary: $59,000 annually (compensation will be based on factors such as experience, education, and program size).

Status: Full-time (35 hours per week).
